Transitional Care, Swing Beds
A swing bed is a service rural hospitals provide allowing a patient to transition from acute care to post-acute care without leaving the hospital. Rather than driving from place to place for different appointments, a swing bed program allows the patient to stay in the same hospital they received acute care treatment in. This provides the care they need in a convenient and familiar setting. Additionally, patients who have had a qualifying stay at another hospital can opt to use the swing bed program at BGH for their recovery, offering the benefit of being closer to home while still receiving comprehensive care.
Eligibility: There are certain requirements to be admitted to our transitional care program. Please contact your primary care provider or our swing bed program coordinator at this extension: (208) 263 – 1441 x 1336.
